NURS3002 ASSESSMENT 2 Outline Report: Root Cause Analysis (RCA) Clinical Incident & Quality Improvement Report Details Length: 1500 words (excluding reference list) (+/- 10%) Weighting: 45% Submission: Turnitin Due Date: Wednesday 26 May 2021 (11 pm) Rationale: The purpose of this assessment is to help you develop the skills of report writing, which is a key skill in professional nursing practice. Task: Building upon activities undertaken in the weekly tutorial sessions, this assessment task involves analysing the root causes of a clinical incident and identifying evidence-based quality improvement strategies to prevent the recurrence of the incident. To complete the report, you will be required to: • Analyse an incident provided on the Blackboard site – See ‘Coroner’s Reports’. • In your analysis, identify the various causes that contributed to the clinical incident. • Use an RCA template to analyse the incident and identify root causes. • After analysing the root causes of the incident: a. Prioritise one nursing-relevant strategy. b. For your chosen strategy, draw upon available evidence to justify solutions or possible strategies to minimise the risk of recurrence of the incident. Structure of the Report Structure your report under these headings and include the information below. • Title: Identify the incident you have analysed. • Introduction (150 words) Clearly state what the clinical incident is, define what root cause analysis is, why using it, what methods you are using (such as Checklist Flip), and a brief overview of what you will address in this report. • Root cause analysis (350 words) Complete root cause analysis process that identifies possible root causes of the incident. The process is two-fold: First, develop a flow diagram to show the key events 2 | P a g e crucial to understanding the incident using a maximum of 6 boxes; Then describe and categorise cause/s and/or contributing factors using the provided template. • Potential intervention (350 words) Prioritise one nursing-relevant strategy that is likely to result in meaningful improvement, which could be implemented within the setting where the incident occurred. These references should be drawn from peer-reviewed discussion or research papers, systematic reviews, evidence-based guidelines, or evidence-based systems and processes published within the last 10 years. Try to reduce the number of references (a maximum of five) from websites. If you do, ensure they are from reputable websites, such as those ending with gov. or org. Non- or low-evidence-based sites, such as Wikipedia, MyDr, Ausmed, Health Direct, or Betterhealth, are not acceptable to use as supporting evidence. Textbooks are not suitable sources to support your nursing-relevant strategy. Your chosen coroner’s report is included in the number of references (you need to use in-text referencing for your chosen coroner’s report ONCE). APA 7 th reference style must be used. For your reference list: • Start on a new page • Consistency, such as the format of DOI, capital, or lower cases • Alphabetical order • Ensure all references that appear in the report are listed in your reference list Additional requirements • Have the first line of each paragraph indented using 1.27 cm. • The word limits in each section are a guide only. • Headings should be used. • Submit assignments as Word documents.
